TIPS FOR USING OUR ONLINE SCHEDULER:
- We use a secure portal that protects your information.
- We need your insurance information to confirm your request. This is to allow us to verify we accept your insurance.
- If you don’t have insurance that we will be filing, just answer “NONE/SELF PAY”.
- If you do enter an insurance, add the plan name or put “unknown”.
- Upload a picture of your insurance card front and back if possible. This ensures we have all the information.
- **Watch for the CONFIRMATION text or email within 24 hours of your request.
- “Urgent work-in” requests: If there are no openings available, we can determine over the phone if you need to be WORKED IN.
POST-SCHEDULING SURVEY: If you have any difficulty with the online scheduler, please complete the survey and give us suggestions that can improve your experience.
